Develop a Clinical Photography Consent Form using the AHIMA guidance (attached). The consent is an informed consent but does not have to contain all the elements of informed consent for treatment and procedure listed in book. Use the AHIMA link containing Clinical Photography Policy to determine what is needed in a clinical photography consent form.
Attached is a sample surgical consent, that should help you with the format. Your consent form can be for a hospital, nursing home or physician practice. It has to contain the elements outlined in the AHIMA sample policy.
This is not a group project so I expect to see some differences, not a copy of your classmates
